Investigation Summary
On February 10, 2012, Employee #1, a 49-year-old stationary engineer, was working for his employer, Alsco, Inc., a linen supply service, according to preliminary information. Employee #1 was attempting to adjust or repair an industrial sheet folding machine (Jensen Classic Folder Model Number "760411"), and a moving part contacted his hand. Immediately, his forefinger was amputated to approximately the first digit and his middle finger was lacerated. Employee #1 was treated, while hospitalized for less than 24 hours at Kaiser Medical Center. On February 10, 2012, Cal/OSHA Santa Rosa received a timely report of the accident, which had occurred earlier that day. The investigation revealed that Employee #1 had placed himself in a danger zone, while adjusting the "air blast" on the energized linen folding machine and that his hand was caught in an unguarded pinch point between a belt and a roller.
