Employee's Finger Is Amputated in Machine

On July 11, 2011, Employee #1, of RR Donnelley, was operating a System Automation and Service brand carton sealing machine (Model Number 708T; Serial Number 06-140). The machine had an electronic eye on one end. The electronic eye actually starts the machine to cycle and is part of the machine operation, rather than any type of safety device. Employee #1 was waiting for the work day to start and leaned on the machine, causing the machine to start the cycle. The bar on the machine caught his right little finger between the roller that advances the boxes and the stationary roller. Employee #1 was taken to Trinity Terrace Park Hospital and treated for the amputation of his finger.
