Employee Is Struck in Head by Barrel Cover, Later Dies

On June 13, 2011, an employee was using a plasma cutter to cut a barrel. The barrel exploded, causing the cover of the barrel to blow off, striking the employee in the head, and causing a severe head injury. The employee later died of his injury.
