Investigation Summary
At approximately 9:00 a.m. on April 26, 2012, Employee #1 was working for a firm that engaged in research in molecular biology. He worked at Building #3A at the firm's site in South San Francisco, California. Employee #1 started work at 7:00 a.m. on April 26, 2012, the day of the incident. At approximately 9:00 a.m., Employee #1 and a coworker, who worked in the same building, went through the lobby of Building 5 and exited through some double doors. They were headed to get breakfast at the cafeteria located in Building 4. Employee #1 said he and his coworker had already walked approximately 15 feet (4.6 meters) away from the double doors of Building 5, when Employee #1 slipped. His left foot went forward, and he landed with his right foot underneath his body. Employee #1 sustained a fracture of his right femur 4 inches (100 millimeters) above his knee. Employee #1 said he was walking behind his coworker, who did not see him slip. According to Employee #1, the walkway was wet, as it was drizzling that morning. Employee #1 was wearing Crocs at the time of the accident. He was taken to Mills-Peninsula Hospital, where he was admitted for treatment. Employee #1 was discharged from the hospital after two days of hospitalization. According to Employee #1, he had sustained a fracture of his right fibula and tibia while in Peru in November 2010. Employee #1 said that his foot would get swollen due to his prior fractures, so he had been wearing Crocs to work. They did not help him when he slipped. The Division did not issue an Accident-Related citation, because the accident was not caused by a violation of Title 8, California Code of Regulations (T8 CCR). No other violation of Title 8 CCR was observed, so no citation was issued.
