Investigation Summary

At approximately 11:45 a.m. on April 20, 2012, Employee #1 was using an eight ft ladder to remove a sign that was on the side of a 40 ft trailer. The sign was riveted on the side of the trailer and weighed approximately sixty lbs. with the size of three ft by ten ft 18-gauge metal steel. The employee was on the fifth step of the standing ladder. Employee #1 dislodged all of the rivets except for the last one on the lower right corner. The metal sign was blown by a gust of wind causing the sign to swing and hit the ladder. The employee fell from the ladder approximately five ft. Employee #1 landed on gravel and pavement onto his left hip. Employee #1 suffered a fractured left hip and a fractured right knee. The employee was taken to an area hospital, where he was treated.
