Investigation Summary
On February 27, 2012, Employee # 1 a masonry was standing on the platform of a mast climber scaffold. An extensible boom forklift which was used to elevated materials up to the scaffold accidently hit the scaffold platform. The platform rotated and hit the block wall under construction which cause Employee #1 to lost his balance and fell from an elevation of 35ft. to the concrete floor. Employee #1 died at the scene.
