Employee's Finger Is Injured by Bagging Machine

At approximately 9:30 a.m. on June 7, 2011, Employee #1 was standing on the bottom rung of a step ladder, while trouble shooting the top portion of a Model Number 009 Multibagger bagging machine (Serial Number 009-500-272), on the north side of the "cut" floor. While on the ladder, Employee #1 lost his balance and reached out to the bagging machine to steady himself. At the same time, a coworker was operating the top portion of the bagging machine, on the east side. The coworker lowered the head and crushed Employee # 1's left little finger. Immediately, Employee # 1 yelled to the coworker to raise the head back up. The coworker raised the head, and Employee # 1 went to the nurse's station. The nurse, who treated and assessed Employee # 1, said his finger looked crushed. Employee #1 was taken to Mary Greeley Medical Center in Ames, IA. His finger was amputated.
