Employee Amputates Finger Tips While Operating Machinery

On April 19, 2011, Employee #1 was operating a coupling starter machine. As he reached into the machine to check to see if the motor was still running, he slipped and caught his fingers in the linear cylinder of the carriage clamp. The tips of his index and middle fingers on his left hand were amputated. Employee #1 was taken to a nearby medical facility, where he was hospitalized and treated for his injuries.
