Investigation Summary
On May 11, 2011, Employee #1 was operating an unguarded APC Dry Ice Reformer (hydraulic power press) when his right hand became caught in its upper and lower dies. There were no witnesses to the accident; however, a coworker responded to Employee #1's call for help. Emergency services were contacted and the coworker cut Employee #1's work glove from his right hand, which contained Employee #1's severed fingers. Employee #1 was then taken by helicopter to Good Samaritan Hospital in Phoenix, Arizona, where he was hospitalized and treated for his injury.
