Employee Found Unconscious with Head Injury

July 25, 2011, an Employee was replacing bearings on a Raymond EASi order picker with the forks elevated approximately 5 feet above the floor and tilted forward. The employee was discovered on the floor in front of the forks, face down and unresponsive. The employee sustained a fatal head injury which appeared to have crushed his skull on the left side of his head. The gash was approximately 5 inches in length. It was unclear what his head struck.
