Employee Falls from Elevation and Sustains Multiple Fracture

At approximately 2:30 p.m. on March 5, 2012 Employee #1 of CC Services Inc., was retrieving a file box from a set of shelves approximately 16 feet high with a stock picker forklift. Employee #1 did not use the harness and inertia reel provided and fitted to the equipment operating station. When Employee #1 raised the platform to the desired height of approximately 12 feet, he walked out on the pallet attached to the forks of the lift. While Employee #1 was searching for the desired file he walked off the end of the pallet, falling approximately 12 feet to the concrete floor below. Employee #1 was hospitalized due to his fractures.
