Investigation Summary
On January 15, 2013 Employee #1 of Alloy Die Casting Co. was moving a stack of aluminum ingots from one area to another. The shape of these ingots allows them to be stacked into an interlocked bundle. In order to place this load on a wooden pallet, he wanted to place two pieces of ingots, on the pallet, in parallel with the forks of the forklift to create opening for the forks to lift up the bundle without the pallet. To align these ingots with the forks, Employee #1 stopped the forklift with forks slightly raised above the pallet. There was no load on the forks and he had turned off the forklift motor and got out of the driver seat. He was placing an ingot on the pallet parallel to the fork, when the forks descended suddenly onto the pallet and his left little finger was caught in between the fork and the pallet. Employee #1 amputated his left little finger up to the bottom of the nail, lost about 1/2 inch of his finger. During the surgery, his little finger was cut further up to the first digit, losing another 1/2 inch of his finger. He was treated as an outpatient and was not hospitalized.
