Investigation Summary
At approximately 12:00 p.m. on November 24, 2012, and incident occurred at a hand tool factory. Employee #1 was using a full revolution punch press machine to punch holes in tool brackets. Employee #1 was hand feeding the brackets, punching them individually. As Employee #1 was punching the bracket, he reached into the point of operation to remove the bracket. The press cycled again unexpectedly, as Employee #1's hand was still underneath the die, so that the die came down on his right hand. The three middle fingers on his hand were crushed and amputated. Employee #1 was taken by emergency responders to a local medical center for treatment of his injuries.
