Employee Crushes Hand in Dough Mixing Machine

On June 28, 2012, Employee #1 was cleaning a Peerless dough mixing machine. The employer operates a bakery that has a large scale pastry production operation. Employee #1 has worked for this employer directly for 24 years. On the day of the incident, Employee #1 removed the machine guards in order to gain access to the dough mixing paddles to clean them, and did not adequately deenergize or disengage the power source with effective lockout/tagout procedures. While attempting to remove dough from inside the mixer, the mixing paddle cycled and crushed Employee #1's left hand. Employee #1 was hospitalized for treatment of a fractured hand.
