Investigation Summary
On June 5, 2012, Employee #1 was cleaning the chill rolls without any plastic sheet running through them. The employee was using a rag to wipe the rolls while they were running. Employee #1's right hand was pulled into the nip point of the in-running rolls. The employee suffered crushing injury to four fingers on his right hand. Employee #1was transported to an area hospital. After about a week, the employee's right ring finger was amputated, and part of his middle and little fingers were also amputated during an outpatient surgery. Employee #1 was not hospitalized.
