Investigation Summary
On May 26, 2012, Employee #1, a 47-year-old male laborer, with Turfevolutions, was working with his supervisor and were attempting to push up the trailer's dump bed to unload dirt left in the bed from the previous day. The battery-powered ram did not have sufficient power to fully raise the bed. Employee #1 reached down to hold in the bed-raising button but the bed was too heavy for the two workers to hold up. The rim of the bed struck Employee #1's right thumb, which was resting on the trailer body rim. The force crushed and amputated the thumb. The coworkers informed the owner, who called for emergency responders. Employee #1 is a direct hire of the employer. The Employer sells, delivers and installs artificial turf at residences and commercial locations.
