Investigation Summary
On December 5, 2011, Employee # 1 a roofer was removing roof tiles from the edge of an 8 foot walkway roof opening, which consisted of a connecting ridge and five exposed 2-in. by 6-in. 16 in. on center decorative rafters. Employee # 1 placed his hand on a rotted rafter, which broke; he lost his balance and fell through the opening approximately 14 feet onto the concrete walkway below. Employee # 1 was transported to the hospital for head trauma injuries. He was hospitalized for two days before he died. The accident was reported to DOSH (Division of Occupational Safety and Health) on December 5, 2011.
