Employee Is Injured in Slip and Fall Incident

On October 4, 2011, Employee #1, an injection mold operator, was cleaning a machine when he slipped and fell. A coworker immediately called 911 when they found Employee #1 lying on the ground holding the back of his head. Employee #1 was transported to Western Medical Center in Santa Ana, CA. Diagnostic testing determined Employee #1 sustained a concussion and was hospitalized.
