Event: 05/26/2011
Employee Is Injured in Fall from Ladder, Later Dies

On May 26, 2011, an employee was seen standing on top of a small parts storage room within the warehouse measuring pipes for a natural gas line that was in need of repair. The employee had accessed the 8-ft. 2-in. structure with a 16-foot extension ladder. A coworker drove by the structure on a forklift and observed the employee standing near the edge of the structure. The coworker delivered a load of metal to the machine department, which was located adjacent to the storage room, within a matter of 5 seconds, when he heard the employee scream and the ladder hit the floor. When the coworker turned, he noticed that the employee was lying on his back on the floor, bleeding from his ears, with his hands in the ladder rungs. After paramedics arrived, the employee was transported to the hospital, where he died from his injuries eight days later.
