Investigation Summary
On March 2, 2011, Employee #1 and his son were under the cab of a 26-ft mail delivery truck repairing an accelerator cable. While still under the truck, Employee #1 directed his son to turn the ignition on. The truck, which was still in reverse gear, moved backwards and rolled over Employee #1's chest. He was killed.
