On June 16, 2011, Employees #1, #2, #3, and #4 were working on a Hek platform performing masonry work. One of the outrigger work platform support brackets broke at the weld. Employees #1, #2 and #3 fell approximately 23 feet to the ground. No other details were provided in the original narrative.
