Investigation Summary
On May 12, 2011, Employee #1, an electrician, was working at a plant that specialized in the titanium-alloy dipping operations. During the dipping process, one of the plants three phases of power was lost. The lights flickered, and their crane, air knives, as well as ventilation stopped working. Employee #1 and another electrician entered the plant area to switch the power over to another substation feed; however, they were unsuccessful. No injury was reported.
