Investigation Summary
On January 24, 2014, Employee #1 was servicing a walk-in cooler at a restaurant. He used a ladder to climb to the top of the cooler and inspect its roof mounted condenser unit. A customer informed one of the restaurant's employees that an individual was outside the cooler and lying next to a ladder. Employee #1 was found at the base of the ladder with head trauma. It is unknown if the employee was climbing up or down the ladder. Employee #1 was transported to a hospital, where he later died.
