Investigation Summary
At approximately 3:00 p.m. on July 2, 2012, Employee #1 was changing a tire on a manure spreader in a field. He did not place the tire in a cage or on the vehicle while filling it with air. As the tire was being filled, it exploded and ejected debris at Employee #1. He was struck and killed by the flying debris.
