Investigation Summary
On May 7, 2011, Employee #1, of Mann Packing Co. Incorporated, placed her hand on the frame member of a conveyor. For some reason, the plastic paddles of the conveyor that close the frame member smashed Employee #1's left index finger. Employee #1 was transported to the hospital, and the tip of the left index finger was surgically removed.
