Investigation Summary
On January 13, 2012, Employee #1 connected a welding machine to a crane and a dolly cart to the welding machine. While they were both been lifted to be placed in a different location, the dolly came loose and struck Employee #1. The dolly fell approximately 50 feet on to the ground. The employee was transported to Coral Gables Hospital where he died at 2:00 p.m.
