Investigation Summary
At approximately 9:27 a.m. on August 16,2011 Employee #1 was run over and killed by a concrete mixer truck that was backing up in the installations of a concrete plant. Employee #1 was waiting his turn to load his truck with concrete. While Employee #1 was waiting, he stepped out of the truck and was standing in the immediate area of another truck, which accidently backed over him, Employee #1 died from his injuries.
