Investigation Summary
At approximately 6:30 a.m. on March 5, 2012 Employee #1 was weighing phosphorous acid in a bucket and he poured the powder into another bucket that contained granular potassium hydroxide, a base. The incompatible chemicals exploded and hit Employee #1 in the face and his eye. Employee #1 was not wearing his splash goggles along with a face shield. Employee #1 used an emergency shower and eyewash for about 15 minutes to flush his face and eyes. Employee #1 was transported to UCSD Medical Center Burn Unit in San Diego for medical attention and he received treatment for three days.
