Investigation Summary
At approximately 1:41 p.m. on December 31, 2011, Employee #1, a tree trimmer, was working for a landscaping company. The employee was to trimming palm trees ranging in size from 18 to 22 ft tall using gaff spikes, a Weaver positioning saddle, a self-locking carabineer, a 120 ft diameter blue lowering rope, a Stihl MS 201 T gas powered chain saw with a 12 in. blade and an orange positioning rope. The employee climbed the tree and cut the palm fronds and seed pods then changed from the orange positioning strap to the blue lowering rope. While hanging from the overhead rope the employee noticed a palm frond that needed trimming. Using a chainsaw the employee cut the rappelling rope resulting in an 18 ft fall onto a property line block wall. The employee was taken to the Tri-City Medical Center, where he was treated for low back pain and a disc compression fracture. Employee #1 remained hospitalized.
