Employee Seriously Injured by Garbage Truck

At approximately 5:30 a.m. on October 6, 2011, Employee #1, a mechanic, was testing the limit switches on a front end loader refuse truck, to determine why the packer wasn't moving out. Employee #1 got up on a six foot fiberglass stepladder next to the side of the refuse truck so he could reach the limit switches and trouble shoot what was wrong with the packer in the bed of the garbage truck. Employee #1 was positioned chest high over the top of the fork arm control levers which move the fork arms up and down on the garbage truck, so trash bins can be pickup and dumped and returned to the ground. Employee #1 slipped off the ladder and fell on top of the control levers causing the fork arms to move towards the front of the cab. Employee #1 was stuck on top of the control levers with his feet dangling off the ground. As the fork arms moved forward to the front of the cab of the garbage truck it pinned Employee #1 causing him to be crushed. Employee #1 was transported by ambulance to a local hospital. Employee #1 was in the hospital for four days for his injuries.
