Investigation Summary
At approximately 10:40 a.m. on September 28, 2011, Employee #1, a laborer, was working in a granite cutting shop, assisting a coworker, who was operating the forklift truck to move a 1,000-lb slab of granite onto an outside storage rack. Employee #1 was standing at one end of the slab while the forklift operator dropped it onto a storage rack. They lost control of the slab, and it tipped off of the rack. Employee #1 stepped in to try to stop the slab from falling, and was pinned between the falling slab and another rack of slabs in storage. He suffered a crushing injury to his lower chest. He sustained internal injuries and was hospitalized for treatment.
