Investigation Summary
On November 5, 2012, Employee #1 was ripping and cutting a wooden section on a table circular saw. There was no safety guard at the circular's saw point of operation. The circular saw blade was also rotating at 3,600 rpm. As he manually feed the stock to the blade, the wooden piece became stuck on the slots of the table. He decided to disengage the wood. While releasing the wood, the fingers of his left hand contacted the saw blade. The blade partially amputated one finger and lacerated two other fingers on his left hand. Employee #1 was admitted to a orthopedic clinic, where he was treated, hospitalized and then discharged.
