Employee Amputates Two Fingers Cleaning Machine

On June 29, 2011, Employee #1 was recleaning the cutting head edge of the Potsticker machine. He did not deenergize the power by following the lock out procedure. As he was cleaning the cutting head edge with a fabric pad, his left hand middle and ring finger tips made contact with the reciprocation cylindrical blade located inside the cutting head. He sustained partial amputation of two fingers and was hospitalized.
