Investigation Summary
On February 16, 2011, Employee # 1 was performing stucco activities from the second tier level of the scaffold perform, approximately 14 feet above the ground. Employee # 1 fell through the opening (16 in. to 18 in.) between the house and the scaffold. Employee # 1 was reaching from the scaffold to the house to take a measurement and fell. Employee # 1 was transported to the hospital, where he later died.
