Employee Suffers Multiple Fractures Falling from Scaffold

At approximately 7:30 a.m. on November 3, 2010, Employee #1 and a coworker were performing carpentry work inside a portico. They erected a tubular welded frame scaffold, three frames high. The foreman stated that the tubular welded frame scaffold was larger than the portico opening. The top platform of the frame scaffold to the hard gravel base was approximately 15 ft. The bottom of the portico was approximately 17 ft above the gravel driveway. The top of the portico was approximately 25.5 ft. They had erected wood L-brackets, which they referred to as California corners on the inside framework for the portico. A Keller, Model Number PA208k aluminum extension plank, was placed across the wood brackets, approximately 18 ft above the gravel driveway. Employee #1 worked from either the frame scaffold platform or the Keller aluminum plank all morning without fall protection. The coworker worked from the ground, cutting pieces of material for Employee #1 to install. The coworker stated that he was cutting material when he heard a big bang and Employee #1 fell to the ground. Employee #1 suffered a concussion, fractures to his face, eye socket, left shoulder, forearm and pelvis. Employee #1 was hospitalized.
