Investigation Summary
At approximately 4:30 a.m. on July 5, 2012, Employee #1 was installing a tire onto a rim. The employee was forcing it onto the rim using a tire machine. The tire burst and struck the Employee #1 in the face and chest. The Employee was transported to Community Regional Medical Center, where he was admitted for injuries to his chest and face. Employee #1 was hospitalized.
