Investigation Summary
At approximately 6:15 p.m. on March 5, 2012 Employee #1 of Harvest Container Company sustained a left leg injury. Employee #1 was pinned between a forklift and a section of the railing when he got off the forklift to get his jacket. Employee #1 failed to set the parking brakes, put it into neutral, and turn the lift off prior to dismounting the forklift. Employee #1 received multiple fractures of the left leg.
