Investigation Summary
At approximately 7:00 a.m. on December 3, 2011, Employee #1, a sanitation technician I for Grimmway Farms, was washing a piece of equipment. The rain suit jacket he was wearing was caught by an unguarded rotating projected shaft, which was located about waist level. Employee #1 was transported to a hospital and treated for a left distal ulnar fracture of his forearm.
