Investigation Summary
At approximately 7:30 p.m. on November 30, 2011, Employee #1 was standing in front of the picker to observe the maintenance process of the spindles being lubricated, when he observed some debris at the bottom of picker head. Employee #1 proceeded to remove the debris by knocking it out using his right boot. Employee #1's boot was caught by the moving spindles and entangled his leg and foot inside the picker head. Employe #1 was transported to a local regional medical center, and admitted for multiple fractures.
