Investigation Summary
On July 8, 2011, Employee # 1 was working for his employer, a painting contractor. He was assigned to prepare the ceiling on the new carwash for painting. As Employee # 1 was walking on the partially disassembled scaffold platform, he fell approximately 10 to 12 feet off the scaffold at the end without guardrail. He suffered unspecified head injuries. Employee # 1 was transported to Fresno Community Hospital. The accident was reported to Cal-OSHA on August 31, 2011.
