Investigation Summary
At approximately 7:15 a.m., on September 6, 2011, Employee #1 of Stamoules Produce Company was struck and trapped between two forklifts. Employee #2, the operator of a Toyota forklift, was loading pallets of cantaloupes into refrigerated trailers/trucks parked at the dock area. As Employee #2 backed out of the trailer to load additional pallets, he did not see that Employee #1 was standing behind him as he backed out. Employee #2 struck Employee #1 pinning him against a second forklift. Employee #1 incurred multiple pelvic fractures.
