Employee's Finger Is Amputated by Transport System

On March 10, 2011, Employee #1, a general manager for plant operations, was adjusting/aligning the irrigation tape going into the splitting section of the production line. The transport system was composed of two double rollers that created an in-running nip point hazard. His hand came into contact with, and was drawn into the nip point, resulting in a crushing injury to his hand, and amputating his finger. Upon inspection of the tape production line, it was found that the nip point hazards and the chain/sprocket drives were not guarded. Employee #1 claimed that he was not adjusting, but showing the engineers the operational problem, and his carelessness caused his injury. It was found that procedures, means, or other methods had not been developed to protect operators from these hazards. In the course of this investigation, it was verified that two previous serious injuries associated with nip point hazards had occurred on different production machines. Employee #1 was not hospitalized for his injury.
