Investigation Summary
On September 3, 2010, Employee #1, a woodworking machine operator, was pushing a 1-inch by 6-inch by 40-inch board through a 10-inch Powermatic table saw, using a 1-inch by 2-inch by 24-inch board as a push stick. He was wearing Atlas FIT gloves made of a cotton/polyester material with the palm coated in natural rubber. The Powermatic saw was mounted on a pallet and equipped with a blade guard and anti-kickback device. As Employee #1 pushed the board through the blade, it split, causing him to fall forward, and his hand made contact with the saw blade. He sustained a partial amputation of his index finger and a laceration to his thumb. He was hospitalized for treatment.
