Employee Falls Down Stairs and Dies

On April 26, 2011, at approximately 1:00 AM, Employee #1 arrived for work and according to the direct supervisor had slurred speech, stumbled about, and could not dress in the appropriate work clothing. Employee #1 was told to wait in the changing room where another manager proceeded to escort the individual from the facility to be sent home. Upon exiting the facility Employee #1 fell down the stairs from the concrete pad suffering blunt force injuries to the head. EMT's at the scene found Employee #1 without vital signs and the individual was pronounced dead.
