Employee Amputates Finger in Package Sealing Machine

On September 13, 2010, Employee #1, a maintenance worker, finished setting up the Sinlon SL-500 Automatic Servo Wrapper machine. Three production employees were nearby and one of them witnessed the incident itself. The sealing jaws of the wrapper machine were heated and the top jaw rotated to seal and cut apart packages of product. The openings for the infeed and outfeed sides of the sealing jaws area were inadequately guarded. The infeed side opening was approximately 10.5 inches high and 5 inches wide and the point of operation of the sealing jaws was approximately 12 inches. The outfeed side opening was approximately 5 inches high and was approximately 11 inches from the point of operation. While observing the machine in normal operation, before product had reached the sealing area, Employee #1 rested his hand on the conveyor next to the opening of the sealing jaws area. Subsequently, Employee #1's hand was fed into the sealing jaws, resulting in an amputation of the last phalange of the middle finger on his left hand.
