Investigation Summary

On June 30, 2012, Employee #1 placed a closed 24 ft extension ladder against a tilt up wall. He then climbed approximately 9 ft and started to position himself as the top of the ladder slid approximately 9 in. down the wall and stopped. While Employee #1 was looked down, he realized the ladder's pitch was unsafe. The ladder continued to slide the rest of the way down. His legs fell through the rungs. When he reached the ground, the ladder slid into his shins and caused him to fall forward onto the ladder and fracture his right leg. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized.
