Investigation Summary
At approximately 9:15 a.m. on August 13, 2010, Employee #1 was working at a meat packing plant. She was on light duty due to a crushing injury to her right ring finger tip and nail earlier. She was a permanent worker and had worked in the area for almost one year, however, not at this position. She was monitoring product movement to ensure it flowed into the slicer. The product drops from the slicer to a horizontal auger which moves it to a vertical auger moving it to the packaging area. She was monitoring the product at eye level when a hook, used to move product and resting in her left hand, was caught in the auger. She suffered an amputation of her left index finger and was hospitalized.
