Employee Is Killed Between Forklift and Door

On September 20, 2010, Employee #1 was operating a narrow aisle forklift. As he approached an automatic door between the docks, the forklift turned to the right quickly, and he was either ejected or stepped from the moving forklift. He struck against the door barricade and was momentarily caught between the still moving forklift and the stationary barricade. Employee #1 was crushed and killed.
