Employee Is Killed Falling Into Unprotected Pit

On September 20, 2010, Employee #1, a maintenance worker, and a coworker were in the process of setting up an evacuator to vacuum out excess grain that had accumulated in the bottom of the boot pit. There was approximately 4 ft of grain to be vacuumed. A 2 ft by 4 ft floor grate was removed from the work floor to gain access to the boot pit area. No barricade or other type of warning device was available around the open pit. The coworker put the parts together needed to reach the bottom of the boot pit and decided that he needed additional help in putting the system together so he called Employee #1, via radio, to come and help. Employee #1 failed to respond to two separate radio requests so they went to find Employee #1. Employee #1 was found in the bottom of the boot pit. He had fallen into the unprotected pit and died.
