Investigation Summary
On September 13, 2011, Employee #1 was using a portable diesel fuel tank to refuel a running John Deere 4040 tractor. As he fueled the tractor, the unmanned tractor began to move. He tried to stop the tractor, but was struck and ran over by it. Employee #1 sustained a traumatic chest injury that killed him.
